Membership

A membership at Dalton Health Direct Primary Care is very straightforward.

You simply pay a monthly membership fee for comprehensive primary care services.

These primary care services include: annual wellness visits, routine annual labs, chronic disease management, same or next day sick or urgent care visits, Telehealth visits, as well as direct access to your provider by phone or secure messaging.

We are transparent in our pricing: there are no hidden fees or co-pays.

We are currently registering patients over the age of 17.

Enrollment requires a registration fee of $99 per household. 

As membership space is limited, billing begins upon enrollment.

A method of payment is required and must be eligible for auto-draft.

You may cancel your membership for any reason with a 30-day notice. All contracts are automatically renewed unless a 30-day notice is received by us not to renew.

Dalton Health is a membership based model and fees are billed monthly.
